Forms and consent
Build forms with rules and scores, then have them signed.
Setup lists every form with its type, version and status, and a full-screen builder edits them. Rules show or hide fields depending on an answer, page breaks turn a form into a wizard, and answers can carry scores.
Fill a form on the patient’s record, hand over a tablet with the staff screens hidden, or send several forms behind one link by email or text.
