Use AI and Slack to route finished visits to aftercare.

Let AI outline each visit, then approve its aftercare sequence from your team’s channel.

Overview

The day after a treatment is when the questions come up. AI outlines the visit, a person approves with one click in Slack, and the patient goes into the aftercare sequence that fits.

Prerequisites

These are the pieces we suggest having ready before you switch this workflow on. Your clinic may set things up differently, so treat the list as a guide.

How to use this template

Once it opens, you’ll see the blocks joined up in order.

  1. The Appointment status changed trigger is the first step. A run starts when a visit’s status changes in your diary, whichever member of the team changes it. Choose the appointment types that have aftercare, and leave out the ones that don’t, so a routine review never turns up in your approval channel.
  2. The Condition block lets through only the visits that have really finished. Set the rule to status is Complete, and a patient who is merely checked in or moved to another room ends here. If only certain treatments carry an aftercare sequence, add those as a further rule, so nothing reaches Slack without something to approve.
  3. The Summarise record block uses AI to outline the visit for whoever approves it. Ask for what the treatment was, anything unusual in the note and what the patient was told to expect, so a clinician can decide without opening the chart.
  4. The Post actions to channel block puts the summary in Slack with a button to approve. Pick the channel your clinicians watch during the day, add the {Summary} variable to the message and name the button after what it does, such as Start aftercare. The run waits here until someone clicks it, which is what keeps a person in charge of what patients receive. You decide how much of this runs without a click: once your clinicians trust a treatment’s sequence, you can remove this block for that treatment alone. An example message input would be:
{Record > Name} finished {Record > Treatment} with {Record > Clinician}. {Summary} Start their aftercare sequence, or leave it with the care team to follow up by phone?
  5. The Add to sequence block starts the patient’s aftercare once it’s approved. Choose the sequence for this treatment, and the first check-in goes out on the timing you set there, with any replies coming back to the patient’s record.
