Create a medical history questionnaire
Build the intake form patients complete before their first visit, with the right questions.
Table of Contents
Go to Setup, then Forms, and click Create Form.
Choose Blank Template, a template, or the AI option.
Set the form's type to Medical History.
On Components, add your questions.
What to include
Current medications and allergies, using the Medications and Allergies components.
Past conditions, using Medical Conditions.
Lifestyle questions that matter for your treatments.
A Signature, if the patient should confirm their answers.
Click Create when you are done. To send it automatically, see the form's settings for requesting it before an appointment.
