Create a treatment form

Build the template clinicians use to record what happened during an appointment.

Table of Contents

Go to Setup, then Forms, and click Create Form.
Start from Blank Template, a saved template or AI.
On Type, choose Treatment Form.
On Components, add your fields. Treatment forms also offer Lab tests, Education and Follow up.
Use Preview to check it, then click Create.

Link the form to services so the right template appears first for each appointment.
