Record a patient problem
Add a coded diagnosis to the chart so the whole care team sees the same active problems.
Table of Contents
Open the patient card and click the EMR tab.
Open Diagnoses.
Click Add problem.
Search for the diagnosis by name or code, such as an ICD-10 code, and choose it.
Add the dates, severity and any notes, then save.
Active problems also appear on the patient's summary.
