Complete medical forms on the app
Fill in and sign a consent or medical history form on a device.

You can fill in a form yourself or pass the device to the patient during a pathway.
Fill it in
From the appointment, tap Forms and choose the form.
Answer the questions. Anything required but missed turns red with Answer the questions marked in red.
Sign in the Sign here box if the form asks for it.
Tap Complete form, or Save draft to finish later.
A few field types say Fill this field on the web app. Complete those on the web.
If you see There are no forms yet., create one on the web under Setup in Forms.
