Set up a lab form

Build the form your team fills in when requesting lab tests, with the right fields.

Table of Contents

Go to Setup, then Forms, and click Create Form.
On Type, choose Lab Form.
Name it and link it to any services.
On Components, add Lab tests, Code Lookup and Allergies, along with the fields your labs need, such as the order number.
Create the form.
