Set up and track outcome measures
Chart a patient's scored responses and vital signs over time, with severity bands.

Mark any field on a form as a measure, such as a weight or a GAD-7 score. Each completed form then adds a point to a chart on the patient's card.
Set up a measure
On the Forms page, click Configure Measurements, or open the form.
Choose the field and name the measure.
Add severity bands, such as mild or severe.
Save.
See the results
Open the patient's EMR and click Measures. You see the trend, the change since the last visit and the change from their baseline.
