Send a form before the appointment
Send forms automatically, so patients can complete them at home ahead of their visit.

Go to Setup, then Forms, and open the form.
On the details tab, choose the services it's for.
Turn on Request to be completed prior to appointment.
Save.
The status column now shows Active. Make sure the confirmation notification is on, because it carries the form link.
