Link forms to services
Connect a form to the treatments it's for, so it appears or sends automatically.

Go to Setup, then Forms, and open the form.
On the details tab, add the services the form should be used for.
Turn on Request to be completed prior to appointment to send it automatically.
Save.
You can also attach forms from the service's Forms tab.
