Create a medical form

Build your own forms, or start from a template.

Table of Contents

Go to Setup, then Forms under Clinical, and click Create Form. Pick a starting point: Blank Template, Generate with AI to convert your existing documents, or Browse Templates for ready-made forms.

Build the form

On Type, pick Medical History, Consent, Treatment Form, Prescription, ePaper or Lab Form.
On Details, enter the Form name and choose the services it applies to.
On Components, drag fields onto the canvas and adjust each one in the settings panel.
Use Advanced to add rules that show or hide fields based on answers.

Check and publish

Click Compliance Check to review the form and Preview to see it as a patient will. Then click Create Form.

Manage your forms

In the list, hover over a row for Preview and View analytics. Its menu offers Edit, Clone, Print and Delete. Click the number under VERSION to see earlier versions.
