Restore a medical form
Bring back an inactive form or an earlier version, without affecting your current version.

Restore an inactive form
On the Forms page, click Filter, choose inactive forms and apply.
Click ⋯ on the form and choose Restore.
Restore a version
Click the number in the Version column to open Version History.
Pick the version and restore it.
