Generate a form with AI
Turn the PDFs, documents and photos of forms you already use into ready-to-edit forms.

Upload a PDF, Word document or photo of a form, and AI rebuilds it with the right fields, such as headings, text boxes, tick boxes and signatures. You can upload several at once, which is a quick way to bring across a whole set.
Go to Setup, then Forms, and click Create Form.
Choose Generate with AI.
Upload your files, or describe the form you need.
Review each result and make any changes.
Publish it.
AI-generated forms don't replace the advice of a qualified professional. Check every form before using it with patients.
