Use a ready-made form template
Start from a form in the library instead of building your own from a blank page.
Table of Contents
Go to Setup, then Forms, and click Create Form.
Choose Browse Templates.
Search, or filter by type and profession.
Click a template to preview it.
Add it to your account and adjust it for your clinic.
Review templates carefully before you use them, as they aren't tailored to your clinic.
